(Boston Scientific CDx) before the patient’s service has started or after the patient’s service has ended will not be processed, reviewed or analyzed by Boston Scientific CDx, and Boston Scientific CDx shall have no liability for any such information or data. You are responsible for collecting all information for the patient’s medical record via ECG Insight™ used for any clinical decision making, and you can collect any such information by utilizing the data request feature for up to 90 days after the patient’s service end date.
