Boston Scientific Cardiac Diagnostics Privacy Policy

Policy scope
This Privacy Policy (Policy) sets forth the Boston Scientific Cardiac Diagnostics, Inc. (“BSC CDx” or “We”), a wholly owned subsidiary of Boston Scientific Corporation (BSC), privacy practices regarding the collection and use of personal information, personal data or other regulated information associated with natural persons (collectively, "personal data") relating to BSC CDx websites, applications, or other digital platforms, including but not limited to mobile sites and our applications, directed to individuals in the United States (collectively, Sites) that link to or otherwise reference this Policy. This Policy also describes your choices regarding our use of your personal data and how you can exercise rights related your information, as applicable, such as to request access, modification, portability or deletion of such information. When we refer to BSC, we mean Boston Scientific Corporation, including our affiliates and subsidiaries. BSC adheres to privacy best practices, including the Generally Accepted Privacy Principles (GAPP). Reporting under this Policy may be directed to BSC Global Privacy.

Protected health information
We are dedicated to maintaining the privacy and integrity of your protected health information ("PHI") that we collect when working with healthcare providers. Patient information shared with us by these healthcare providers or received by us on their behalf will be used and disclosed consistent with the Health Insurance Portability and Accountability Act and our contracts with those providers and is not subject to this Privacy Policy.
Information collection and use
Personal data is data that reasonably relates to or can be used to identify or contact you or another member of your household, such as your name, email address, telephone number or similar information. BSC collects personal data you voluntarily give us, such as but not limited to, when you request information via our Sites, submit your resume, register for Sites or events, sign up for communications or interact with BSC team members.

Our Sites include interactive digital health tools, including enrollment portals, through which information is collected from users. We also collect information about your interactions with our Sites. We collect such information using web logs, cookies, pixels, beacons and similar tools, some of which may be operated by third parties. Our Sites automatically track certain behavior (usage data) during visits, such as, referring/exit URLs and we also collect other basic information about you which does not directly identify you but which may correspond with you or a particular device. We use this information to learn more about how our Sites and online resources are used and to otherwise improve and administer the Site. We also use this information to enable us to deliver information tailored to your interests and preferences, based on your use of the Site. For example, we may collect the internet protocol address assigned to your computer by your internet service provider. This address may change each time you connect to the internet (a “dynamic” IP address), or it may remain the same (a “static” IP address). In most cases, this information is collected automatically, for our legitimate business interests. Licensed medical professionals must provide additional personal data to register (e.g., profession, title, Medical ID and clinical affiliation) in case of Sites with restricted access. In addition to the uses mentioned above, we use this data to review and verify your registration. We may obtain additional information about you from other sources through your Unique Physician Identification Number (UPIN).

Cookies and similar technologies
We use, and allow certain other companies to set, cookies and similar technologies on the Sites. BSC deploys physical, technical, and procedural safeguards designed to protect the personal data we collect. However, we cannot guarantee the security of your information, and the transmission of information via the internet, including by email, cannot be guaranteed.
